The Joint Commission survey is clinically based, however, it all comes down to what is documented in the medical record. Medical record personnel as well as clinical personnel need to attend this conference. Discussion will include the medical record review process and authentication requirements (including verbal orders). Documentation for challenging standards will also be discussed. These include:

  • Assessment and reassessment
  • Moderate and deep sedation
  • Pre- and postoperative procedures
  • Restraint and seclusion

Remember, Joint Commission is reviewing more medical records during the survey process.
